NCR to CAPA / 8D checklist https://getshopfloorai.com/ncr-capa-8d-checklist Use this to draft a CAPA or 8D from an NCR you already wrote: confirm the record, choose whether it stays an NCR, and keep a person at every decision. The matching directory entry names n8n and Claude. Treat the steps below as a practical pattern. They have not been timed in a shop, and they do not replace your quality procedure. Before a draft is allowed - NCR number, date, source, and owner are filled. - The requirement is cited, with its revision. - The actual condition is what was measured, seen, or returned, with no guessed cause. - Affected quantity and suspect quantity are separate. - Part, revision, lot, work order, and location are on the record. - Containment already taken is written, including who was told. - Evidence references point at a photo, inspection row, certificate, email, or hold tag. - If the requirement or the actual condition is missing, do not draft. Choose the record - Minor, isolated, and corrected immediately inside an approved method: close on the NCR. - Repeat, unclear cause, or a detection failure or escape: root-cause analysis and a corrective action. - A formal response was requested: CAR. - Supplier-caused, and the supplier must answer: SCAR. - Customer complaint or return, high risk, or a cross-team problem: full 8D. - Link any later record to the NCR number, and write the choice down. Draft and stop at each gate - Every model block is labelled draft. - D1 through D3 restate the NCR only. The quality lead confirms them. - D4 asks 5 Whys and 6M questions and does not state a root cause. The team verifies occurrence and escape against evidence. - D5 and D6 list rework, scrap, repair, return, use-as-is, and concession, and leave the choice blank until a named authority approves. - D7 asks where else the cause could exist. A person decides the scope. - D8 is an unchecked closure list. Implementation is not closure. Keep the three actions apart - Correction changes the product you already have. - Corrective action changes the cause so the same miss does not recur. - Preventive action, or read-across, looks for that cause on similar parts, tools, or steps. - Scrap, rework, or a training note is not a completed corrective action by itself. Close on an effectiveness check - Schedule the check when the action is approved. - Name who looks, at what, and what result sends the cause back. - Set the interval in your own procedure. A 30-, 60-, or 90-day look is only a vendor-described practice. - Do not close because a draft exists or because the parts were scrapped. - A calibration certificate can be evidence. Filing it is not closure. This checklist does not decide disposition, name a root cause, or close a record. It does not set your effectiveness interval. Count overdue NCRs and repeats on your own log. Directory entry: https://getshopfloorai.com/use-cases/ncr-to-capa-8d-draft
